Here are some templates that may help you.  They are provided in both Microsoft Word and Google Docs. To use the Google Docs version, you will need to save a copy before using it.

District Report Sample  (WORD)  (Google Doc)
     This form can be used by District Officers, Chairmen, and Unit Presidents for their reports at District meetings.
